2. Next screen: enter the amount that you would like to pay and the security code and click continue.
3. Final screen: Enter the PATIENT’S NAME OR ACCOUNT NUMBER in the PO Number field. Enter credit card number and billing information. Enter Patient’s name in Shipping info box if it is different from the billing information.
